Однако, у больных с сопутствующей портальной гипертензией цирротического генеза, осложненной состоявшимся ранее варикозным пищеводно-желудочным кровотечением, назначение антикоагулянтов и дезагрегантов может отрицательно повлиять на эффективность гемостатических мероприятий при рецидиве геморрагии, что чревато фатальными последствиями.Клиническое наблюдение демонстрирует результат проведения операции TIPS/ТИПС, дополненной эмболизацией путей притока к пищеводно-желудочным вариксам, пациенту с эндоскопически установленным высоким риском рецидива варикозного кровотечения на фоне имевшейся у него ИБС с нестабильной стенокардией. Достижение эффективной портальной декомпрессии позволило назначить больному антитромботическую терапию, в дальнейшем выполнить коронарное стентирование и продолжить необходимую терапию с хорошим отдаленным результатом по течению обоих заболеваний.Из общего количества 307 больных с осложненной портальной гипертензией, подвергшихся операции TIPS/ТИПС в хирургической клинике РостГМУ в 2007-23 гг., у 27 пациентов портосистемное шунтирующее вмешательство было выполнено на фоне необходимости проведения антитромботической терапии в связи с коморбидной патологией.</p></abstract><trans-abstract xml:lang="en"><p>Antithrombotic therapy in patients with unstable angina is considered mandatory. However, in patients with concomitant cirrhotic portal hypertension, complicated by previously occurring variceal esophagogastric bleeding, the prescription of anticoagulants and antiplatelet agents negatively affects the effectiveness of hemostatic measures in case of recurrent hemorrhage and can lead to poor outcome.Case report demonstrates result of TIPS procedure, supplemented by embolization of inflow tracts to the esophagogastric varices in a patient with an endoscopically established high risk of variceal re-bleeding against the background of coronary heart disease with unstable angina.
