Зернисто-клеточная опухоль (ЗКО) – редкое доброкачественное новообразование, происходящее из шванновских клеток. Она оставляет около 1% всех доброкачественных опухолей пищевода и имеет потенциал к злокачественной трансформации, при этом даже небольшие опухоли (<10 мм) могут быть злокачественными. В КФХ имени Н.Н. Бурденко Первого МГМУ им. И.М. Сеченова 4 пациентам была выполнена эндоскопическая подслизистая диссекция (ESD) опухолей пищевода размерами <20 мм. По результатам гистологического исследования диагностированы ЗКО. ESD является предпочтительным методом удаления опухолей до 30 мм. Для удаления маленьких опухолей (<10 мм) может применяться EMR (эндоскопическая резекция слизистой); опухоли размерами 20-30 мм, особенно с признаками прорастания в мышечный слой, удаляют посредством подслизистой тоннельной резекцией (STER). Хирургическое лечение показано при опухолях >35 мм и признаках инвазии в мышечный слой. Из-за риска малигнизации, а также необходимости дифференцировки злокачественных подслизистых новообразований таких, как ГИСО, ЗКО пищевода подлежат удалению. Выбор метода лечения зависит от размера, локализации и глубины инвазии опухоли, определяемой с помощью эндосонографии.

Abrikossoff’s tumor is a rare benign tumor originating from Schwann cells. It accounts for 1% of all benign esophageal tumors and has a potential for malignancy. Even small tumors (<10 mm) can be malignant. At the N.N. Burdenko Central Clinical Hospital of the I.M. Sechenov First Moscow State Medical University, endoscopic submucosal dissection) was performed on 4 patients with granular cell tumors (GCT) of the esophagus <20 mm. ESD is the preferred method for removing tumors up to 30 mm. For very small tumors (<10 mm), EMR (endoscopic mucosal resection) can be used; for tumors 20-30 mm in size, especially with signs of invasion into the muscular layer, submucosal tunneling endoscopic resection (STER) is an option. Surgical treatment is indicated for tumors >35 mm and/or signs of invasion into the muscular layer. Due to the risk of malignancy and the need to differentiate them from other malignant submucosal neoplasms, such as GIST, esophageal GCTs should be removed. The choice of treatment method depends on the tumor’s size, location, and depth of invasion, as determined by endoscopic ultrasound.
