Выявление факторов риска геморрагических осложнений позволит уменьшить частоту их возникновения за счет планирования хирургической тактики и медикаментозной поддержки пациента, а также ассоциированных осложнений.Цель: выявление и анализ факторов риска развития кровотечений в периоперационном периоде у пациентов с ХИУПК.Методы: ретроспективный анализ историй болезни и амбулаторных карт 766 оперированных пациентов по поводу ХИУПК с 2006 по 2020 гг. В исследование были включены все пациенты, оперированные по поводу ХИУПК. Критериями исключения являлись: острый коронарный синдром, острая ишемия нижних конечностей, инсульт, сахарный диабет I типа, выраженные гнойно-некротические поражения нижней конечности, требующие выполнения ампутаций. Период наблюдения: госпитальный.Результаты: у 44 (5,74%) пациентов наблюдались геморрагические осложнения в раннем послеоперационном периоде. В ходе выполнения бинарной логистической регрессии, наиболее значимыми факторами выступили: эндоваскулярное вмешательство плечевым и бедренным доступом (p = 0,0001), гибридное вмешательство (р = 0,0001), длительность хирургического вмешательства более 60 минут (р = 0,001), тройная антитромботическая терапия (р = 0,001), выраженный кальциноз сосудистой стенки (р = 0,023).Заключение: выявление наиболее частых факторов риска геморрагических осложнений путем всестороннего анализа клинико-демографических и инструментальных данных, а также изучение влияния их на прогноз пациентов с ХИУПК, позволяет выработать персонифицированный подход к лечению таких пациентов и предотвратить развитие указанных осложнений</p></abstract><trans-abstract xml:lang="en"><p>Backgraund: hemorrhagic complications develop with a frequency of 2.0 to 10.5% of cases in the perioperative period in the group of patients with chronic limb threatening ischemia (CLTI). Identification of risk factors for hemorrhagic complications will reduce the frequency of their occurrence by planning surgical tactics and medical support for the patient, as well as associated complications.Aims: identification and analysis of risk factors for bleeding in the perioperative period in patients with chronic limb threatening ischemia.Methods: a retrospective analysis of the medical histories and outpatient records of 766 patients with chronic limb threatening ischemia who underwent surgical treatment (from 2006 to 2020) in order to identify hemorrhagic complications and possible risk factors for their development. All patients undergoing surgery for CLTI were included in the study. Exclusion criteria were: acute coronary syndrome, acute ischemia of the lower extremities, stroke, diabetes mellitus type I, severe purulentnecrotic lesions of the lower extremity requiring amputations. Observation period: in-hospital.Results: 44 (5.74%) episodes of hemorrhagic complications in the early postoperative period.
