Составляя не менее трети от всех ранений груди и живота, данная патология сопровождается высокой летальностью, а также значительным риском развития послеоперационных осложнений. Подходы в лечении таких пациентов претерпели изменения от периода абсолютно консервативного ведения до тактики контроля повреждений и многоэтапного хирургического лечения. Опыт оказания помощи раненым во второй половине ХХ века позволил усовершенствовать анестезиологическую помощь, обеспечить глубокое понимание течения травматической болезни и роли противошоковой терапии и, в конечном итоге, формированию комплексного лечебного подхода с дифференцированным выбором хирургической тактики. В статье обобщены данные о современных методах диагностики торакоабдоминальных ранений, в том числе, на этапах медицинской эвакуации. Приводится частота повреждения органов смежных областей и мнения исследователей относительно этапности выполнения хирургических пособий. Дана краткая характеристика различных видов оперативных вмешательств на органах груди, живота, включая диафрагму, выполняемые на этапах медицинской эвакуации. Представлены наиболее часто встречающиеся осложнения со стороны плевральной полости и живота.</p></abstract><trans-abstract xml:lang="en"><p>Treatment of thoracoabdominal wounds is an urgent task of injury surgery in peacetime and wartime. Accounting for at least a third of all chest and abdominal wounds, this pathology is accompanied by high mortality, as well as significant risks of postoperative complications. Approaches to the treatment of such patients have undergone changes from a period of absolutely conservative management to damage control tactics and multi-stage surgical treatment. The experience of providing assistance to the wounded in the second half of the 20th century made it possible to improve anesthesiological care, provide a deep understanding of the course of traumatic illness and the role of anti-shock therapy and, ultimately, the formation of an integrated therapeutic approach with a differentiated choice of surgical tactics. The article summarizes the data on modern methods of diagnosis of thoracoabdominal wounds, including at the stages of medical evacuation.
