Портальная гипертензия в свою очередь приводит к расширению коллатеральных портосистемных вен вдоль желудочно-кишечного тракта (ЖКТ). Наиболее частым и грозным осложнением портальной гипертензии является варикозное расширение вен пищевода и кровотечение из них. Менее известным, но не менее опасным, является кровотечение из варикозно-расширенных эктопических вен нижнего отдела ЖКТ. Частота развития эктопических варикозно-расширенных вен встречается у 38–56% пациентов с циррозом. Подтверждение диагноза не редко затруднено, а кровотечение иногда бывает массивным. Это может быть опасно для жизни, если не поставить четкий диагноз и своевременно не лечить. В отличие от лечения варикозно-расширенных вен желудка и пищевода, в настоящее время не существует окончательного стандартизированного метода лечения из-за недостаточности данных и редкости.В данной статье мы сообщаем о наблюдении 78-летней женщины с циррозом печени и кровотечением из варикозно-расширенных вен прямой кишки, рефрактерным к медикаментозной и эндоскопической терапии и успешно вылеченного с помощью трансъюгулярным внутрипеченочным портосистемным шунтированием (TIPS).</p></abstract><trans-abstract xml:lang="en"><p>Liver cirrhosis is one of the factors in the development of portal hypertension. Portal hypertension leads to dilation of collateral portosystemic veins along the gastrointestinal tract. The most common and serious complication of portal hypertension is varicose veins of the esophagus and bleeding from them. Less known, but no less dangerous, is bleeding from varicose veins of the lower gastrointestinal tract. The incidence of varicose veins occurs in 38–56% of patients with liver cirrhosis. Confirming the diagnosis is often difficult, and bleeding is sometimes massive. It can be life-threatening if not clearly diagnosed and treated promptly. Unlike the treatment of gastric and esophageal varices, there is currently no definitive recomended treatment due to insufficient data and rarity.
