Внедрение в клиническую практику стратификации риска ранней смерти от ТЭЛА позволило во многом оптимизировать лечение этих пациентов. Однако, сохраняется неопределенность в выборе объема терапии у больных умеренного риска ранней ТЭЛА-ассоциированной смерти. Это лежит в основе не всегда адекватной тактики лечения и, как следствие, развития у данной группы пациентов хронической тромбоэмболической легочной гипертензии, чаще чем у остальных больных. Многие исследователи видят пути решения этой проблемы в установлении новых ЭхоКГ критериев тяжести течения ТЭЛА. Проанализированы современные литературные данные и результаты собственных изысканий по рассматриваемой проблеме. Показано превосходство определения градиента давления на клапане легочной артерии, рассчитываемого на основании измерения объема сердца выбрасываемого в аорту за одну систолу при ЭхоКГ, над большинством широко используемых параметров</p></abstract><trans-abstract xml:lang="en"><p>Predicting and preventing the development of death and chronic thromboembolic hypertension in patients with pulmonary embolism is one of the urgent problems of modern medicine. The introduction into clinical practice of stratification of the risk of early death from pulmonary embolism has made it possible to optimize the treatment of these patients in many ways. However, uncertainty remains in the choice of the volume of therapy in patients with moderate risk of early death. What underlies the often incorrect choice of treatment tactics and, as a result, the development of chronic thromboembolic pulmonary hypertension in these patients, more often than in other patients. Many researchers see ways to solve this problem in establishing new echocardiographic criteria for the severity of pulmonary embolism. This article analyzes modern literary data and the results of their own research.
